Styku Healthpass MSA — Revenue Share Structure

Summary

Partners earn different revenue-share rates depending on whether a sale is attributed to their Affiliate Link or simply attributed to their location through the broader platform. Affiliate Link sales yield significantly higher payouts — 70% of net revenue per Preventative Body Scan, $150 per Healthpass Membership scan, and $10 per Blood Test. Non-affiliate platform sales attributed to the Partner's location pay a flat $10 per completed scan, membership scan, or delivered blood test.

How Styku will share revenue and payout to you:

For all sales completed through the Affiliate Link:

• 70% of net revenue for each completed Preventative Body Scan ($104.30 minus applicable fees of MSRP $149).
• $150 for each Healthpass Membership with a completed scan.
• $10 for each Blood Test upon delivery of the test.

For sales completed on the Healthpass Platform not via Affiliate Link, if attribution to your location is possible:

• $10 for each completed Preventative Body Scan.
• $10 for each scan completed as part of a Healthpass Membership.
• $10 for each delivered Blood Test.
