Yes. Styku provides a Release of Liability Waiver template you can have clients sign before their first scan. It serves the dual purpose of a liability release and a pre-scan consent / acknowledgment form, and it's editable so you can adapt it to your facility.

How operators typically use it
- Fitness, med-spa, and wellness facilities use the template as-is or with light edits (business name, branding) as a one-time client intake / consent form.
- Clinical and medical settings typically customize the Word version — add facility-specific consent language, HIPAA notices required by your own covered-entity status (Styku itself is not a HIPAA-covered entity; see related article on HIPAA vs. consumer privacy), or any additional informed-consent disclosures required by your compliance program.
- Keep signed copies on file per your own retention policy. Styku does not collect or store these forms.
What the template covers
The waiver acknowledges the inherent risks of physical activity, confirms the client is participating voluntarily, and releases the facility and Styku from liability for injury, illness, or property damage arising from the scan or related activities. Review the document for the exact text.
What it does not cover
The Styku template is a general-purpose liability release. It is not a substitute for:
- Medical informed consent specific to a clinical procedure your facility performs.
- HIPAA authorization forms if your facility is a HIPAA-covered entity processing PHI as part of your own practice (Styku scan data itself is not PHI — see related article).
- State- or country-specific consent requirements for biometric data collection (e.g., Illinois BIPA, EU GDPR explicit consent). If you operate in a jurisdiction with biometric-specific consent laws, consult your own counsel.
If you need additional consent language beyond what the template provides, edit the Word version directly or layer it onto your existing intake forms.
